Tormentill for active ulcerative colitis—a pilot study.

BACKGROUND: Extracts from Tormentillae rhizoma (TR) have anti-inflammatory and anti-secretory effects. Because of promising effects in single patients with ulcerative colitis (UC) we investigated tolerability, safety and effectivity in patients with this disease.

METHODS: 16 patients with active UC (clinical activity index (CAI) ³5) were included and received TR subsequently in the doses 1200, 1800, 2400 and 3000mg per day for three weeks respectively. Each therapy phase was followed by a phase without therapy. Outcome parameters were tolerability, side effects, CAI and C-reactive protein (CRP).

RESULTS: Tolerability of all doses was rated as good. Discomfort in the upper abdomen occurred in 8 patients (50%) but was only mild and did not require discontinuation of the medication. Safety parameters remained unchanged. Mean CAI and CRP improved from 8.3±2.8 and 13.0±14.3 at baseline to 3.9±2.6 and 8.3±13.5 during intake of 2400mg TR per day. During therapy 11 patients (69%) came into a complete remission (CAI ²4). In each phase without therapy the CAI deteriorated. 3000mg had no additional effect. Concomitant medication with aminosalicylates was unchanged and steroids could be reduced during the study.

CONCLUSION: TR is a safe and promising new option for the treatment of active UC, which should be evaluated in a controlled study.
